The Let It Fly Official Team Roster form must be filled out by all teams participating in a Let It Fly 4on4 Flag Football tournament .  This form must be handed in to a Let It Fly staff member prior to the team's first game.  Teams that fail to hand this form in will be disqualified from the tournament.  No refunds will be issued to teams that are disqualified from a tournament. 

Please note:  This form requires the signature of the parent or legal guardian of anyone under the age of 18 so should be filled out prior to check-in.
